UT Southwestern Medical Center reported 19 surgical “never events” from July 2024 through December 2025, the highest total among Texas facilities that filed reports.
Objects left in patients after surgery accounted for 18 of those reports. The remaining report involved the wrong surgery. These errors are among the surgical incidents tracked in Texas data on preventable adverse events.
Hospital spokesperson Russell Rian said five retained-object reports were erroneous: three duplicates and two clerical errors. He said UT Southwestern uses a reporting process it considers broader than those of other institutions. The hospital’s account leaves the final count of retained-object incidents lower than its reported figure, but the evidence does not establish a revised statewide ranking.
U.S. News & World Report ranks UT Southwestern first among Dallas-Fort Worth hospitals and second in Texas.
